
First Visit Information
Days Before:
To help you get the most out of your first physical therapy session, we recommend coming prepared with a few key items and details. Here’s what to bring and consider:
1. Questions & Concerns
- Write down any questions you’d like to ask your therapist.
- Think about your goals for therapy and what you hope to achieve.
2. Your Symptoms
List the symptoms you’re experiencing, and include:
- When they started and how long they’ve lasted
- What makes them better or worse (e.g., activity, rest, specific positions)
- Whether they’re more noticeable at certain times of day
3. Medical History
Even if it doesn’t seem related, bring notes on:
- Past injuries or surgeries
- Chronic conditions
- Any recent life changes or events that could affect your health
4. Medications & Supplements
- Create a list of all prescription and over-the-counter medications, vitamins, and supplements you currently take.
5. Medical Documents (If Available)
- Bring lab reports, imaging results, or any diagnostic documents related to your condition.
6. Provider Contacts
- Bring names and contact info for your primary care doctor and any specialists you’d like us to coordinate with regarding your care.
Day Of:
After all your pre-visit prep, here’s what to know as you head to your first appointment at Fortify.
- Arrive 15 minutes before the scheduled time of your first visit.
- Wear loose fitting clothes and comfortable footwear (athletic is the best)
- Give the front desk your ID and insurance card so a copy can be made.
- Complete any other paperwork that is needed prior to your evaluation.
- Ask any questions that you have about insurance or financial concerns.
